Case Study:

NHS Vanguard Care Model

The Challenge:

Guy’s & St Thomas’ Trust and Dartford & Gravesham Trust were going to need to work together as a Vanguard site for the first time to deliver a New Models of Care programme. The model aimed to allow the two trusts to collaborate and share information more effectively, greatly improving patient experience and clinical outcomes. This care model had not been tried before and therefore there was no prior learning for the boards to draw upon. There was an imperative need for the two trusts to be aligned around a single view of how they were going to work together.

Delta7’s role:

• Created a visual tool to be used in workshops with members of the boards.

• Supported the two trust boards to work together in sense making sessions to consider what a collaborative partnership would look like, by describing how their shared values would “build a sustainable partnership that will extend long into the future and can be replicated across the NHS”

• Created a Big Picture tool that has been used to engage with employees of both trusts and a wider group of stakeholders

• Created a video presentation of the Big Picture:

Outcomes

• Increased board understanding and alignment on what collaborative partnership would look like.

• Cross organisational and collaborative learning and increased awareness of collaborative partnership.
